Introduction: Surgical procedures involve risks to patient safety, associated with preventable adverse events, such as errors in positioning and anesthesia. The operating room nurse plays a fundamental role in ensuring safe care during the preoperative, intraoperative, and postoperative periods. The adoption of safe surgery protocols reduces complications; however, their implementation requires the involvement of the multidisciplinary team, effective leadership, and institutional support to achieve better care outcomes. Objective: To analyze the challenges in the implementation and execution of the Safe Surgery Protocol steps from the perspective of Brazilian healthcare professionals. Method: This is an integrative literature review conducted in the SciELO, BVS, LILACS, and Google Scholar databases, considering publications from the last five years. Twelve articles were included, which highlighted recurring difficulties such as low adherence to the checklist, communication failures, professional resistance, lack of training, and weaknesses in the institutional safety culture. Results: The findings show that the nursing team plays a central role in the operationalization of the protocol; however, they face structural and behavioral barriers that compromise the effectiveness of the actions. Conclusion: It is concluded that the proper implementation of the Safe Surgery Protocol depends on effective communication within the multiprofessional team, the strengthening of the safety culture, management support, and continuing education, in order to reduce adverse events and improve the quality of care.
